initial visit
As our patient, your treatment will begin with an examination, diagnosis, and treatment plan. You may have been referred to us for an evaluation of a specific problem or complaint; however, our examination will be comprehensive to insure that we recommend a course of treatment that will be most beneficial to you. When surgery is indicated, it is done either in our offices or in the hospital. We always provide your family dentist or physician with a copy of the treatment you have received with our recommendations for continued care.
Please bring the following information to your first appointment:
- Photo identification
- Referral slip
- Current x-rays
- List of allergies and current medications
- Dental and medical insurance cards
- Name and contact information for your physicians
If your dentist or physician has taken x-rays, you may request that they forward the x-rays to our office. They may either mail or email the x-rays to [email protected]. If additional x-rays are necessary, they will be taken at our office.
IMPORTANT : Please note that all patients under the age of 18 years must be accompanied by a parent or legal guardian.
